PLEASE READ CAREFULLY BEFORE SIGNING, THIS IS A RELEASE OF LIABILITY AND WAIVER OF CERTAIN LEGAL RIGHTS.

In consideration for my being permitted by X Health & Fitness at the Platte Canyon School District Building or property OR Platte Canyon Community Church or in the various recreational activities that take place whether in an indoor or outdoor environment in rough or mountainous terrain, the following is agreed to:

I AGREE TO THE FOLLOWING WAIVER AND RELEASE. CERTAIN RISKS ARE INHERENT IN ANY RECREATIONAL ACTIVITY, PHYSICAL TRAINING, EXERCISE AND WORKOUTS AND CANNOT BE ELIMINATED OR REDUCED. THE SAME INHERENT RISKS THAT CONTRIBUTE TO THE UNIQUE CHARACTER OF THE ACTIVITY CAN BE THE CAUSE OF DAMAGE TO THE PERSON OR EQUIPMENT ASSOCIATED WITH THE ACTIVITY, OR INJURY, ILLNESS, PARALYSIS OR DEATH. I UNDERSTAND AND ACKNOWLEDGE THAT THERE ARE RISKS, HAZARDS AND DANGERS THAT ARE INHERENT AND INTEGRAL TO THESE ACTIVITIES WHICH TAKE PLACE IN A MOUNTAIN / WILDERNESS OR OUTDOOR ENVIRONMENT.

1.Risks are involved in Physical training therefore, I believe and affirm that I am in good physical condition to begin training and have consulted a physician if I am in doubt.

2. I understand that I must be in control of my own actions by monitoring my own physical exertion and identifying the Risks arising out of any activity involving physical exertion.

3. I acknowledge that the above description of inherent risks is not complete and that there are other risks, hazards and dangers associated with participating in recreational activities in an outdoor environment, that may be unknown or unanticipated. Due to the Mountain environment, weather can be unpredictable. As a participant I fully understand the dangers and threats of severe weather.

The inherent and other risks, hazards and dangers may result in damage to the person or equipment, or injury, illness paralysis or death. I acknowledge that the X Health & Fitness had made these risks known to me and I fully assume the risks associated with these activities. I understand that I have responsibilities as a participant in these activities to behave and or act in a reasonable manner for
the type of activity.

I AM VOLUNTARILY PARTICIPATING IN THE RECREATIONAL ACTIVITIES OF PHYSICAL TRAINING IN A MOUNTAIN ENVIRONMENT, WITH THE FULL KNOWLEDGE OF THE RISKS, HAZARDS AND DANGERS INVOLVED AND HEREBY ASSUME AND ACCEPT ANY AND ALL RISKS OF DAMAGE, INJURY, ILLNESS, PARALYSIS OR DEATH.

I, for myself, my heirs, successors, executors and subrogates, hereby KNOWINGLY AND INTENTIONALLY WAIVE AND
RELEASE, INDEMNIFY AND HOLD HARMLESS the X HEATLH & FITNESS, THE Platte Canyon School District, CrossFit Aspen Park, and/or Platte Canyon Community Church and other participants who may be involved in the activities whether located in
Park County, Colorado or in another location.
